Covid-19 Travel Testing Terms and Conditions
Please read the terms and conditions below and confirm you agree to them before booking your appointment.
1. I do not have new or unexplained Covid-19 related symptoms (Fever, Shortness of breath, Cough or barking cough, Decrease or loss of taste or smell, Sore throat, Difficulty swallowing.
2. Runny or stuffy/congested nose, Diarrhea and/or vomiting, Muscle aches)
3. I have not come into contact with confirmed cases of Covid-19 in the last 10 days
4. I have not been advised by Public Health or a healthcare provider to self-isolate
5. I am not covered by AHS (Alberta Health Services) for testing per the guidance at https://www.albertahealthservices.ca/topics/Page17058.aspx
6. I have not been confirmed positive for Covid-19 using a laboratory test or an Antigen test device in the last 90 days
7. I agree to receive communications from Spectrum Diagnostics / Calgary Trail Family Medical Clinic regarding my appointment, screening, results, service or product updates and information, and other communications. This may include phone, email and text message.
8. I agree that any guarantee or warranty for services provided by Spectrum Diagnostics / Calgary Trail Family Medical Clinic is limited to the cost of the tests paid by me
9. I agree that it is my responsibility to confirm travel requirements for my destination with corresponding authorities, and book an appropriate service
10. I agree that Spectrum Diagnostics / Calgary Trail Family Medical Clinic is not responsible for advising me on travel or testing requirements, and to not hold liable for any compensation or damages resulting directly or indirectly from travel delay, interruption, or cancellation for any reasons including delays in reporting of results due to factors outside of Spectrum Diagnostics / Calgary Trail Family Medical Clinic control.
11. I agree to the following cancellation and refund policy: If you would like to cancel your appointment, please note that Spectrum Diagnostics / Calgary Trail Family Medical Clinic require at least 24 hours’ notice. If you cancel within 24 hours of your appointment, you will be charged or if you do not show up for your appointment and have not contacted us to cancel it, we will need to charge you a $50 cancellation fee for a PCR test and a $25 cancellation fee for a Rapid Antigen Test.
We ask that you please reschedule or cancel at least 2 hours before the beginning of your appointment or you may be charged a cancellation fee.
